Periodontal diseases as bacterial infection

The periodontal disease is conformed by a group of illnesses affecting the gums and dental support structures. They are caused by certain bacteria found in the bacterial plaque. These bacteria are essential to the onset of illness; however, there are predisposing factors in both the host and the microorganisms that will have an effect on the pathogenesis of the illness. Periodontopathogenic bacterial microbiota is needed, but by itself, it is not enough to cause the illness, requiring the presence of a susceptible host. These diseases have been classified as gingivitis, when limited to the gums, and periodontitis, when they spread to deeper tissues. Classification of periodontal disease has varied over the years.The one used in this work was approved at the International Workshop for a Classification of Periodontal Diseases and Conditions, held in 1999. This study is an overview of the different periodontal disease syndromes. Later, the systematic use of antibiotic treatment consisting of amoxicillin, amoxicillinclavulanic acid, and metronidazole as first line coadjuvant treatment of these illnesses will be reviewed.

Las infecciones periodontales son un conjunto de enfermedades localizadas en las encías y estructuras de soporte del diente. Están producidas por ciertas bacterias provenientes de la placa bacteriana. Estas bacterias son esenciales para el inicio de la enfermedad, pero existen factores predisponentes del hospedador y microbianos que influyen en la patogénesis de la enfermedad. La microbiota bacteriana periodontopatógena es necesaria pero no suficiente para que exista enfermedad, siendo necesaria la presencia de un hospedador susceptible. Estas enfermedades se han clasificado en gingivitis, limitadas a las encías y periodontitis, extendidas a tejidos más profundos. La clasificación de las enfermedades periodontales ha ido variando a lo largo de los años y es en el International Workshop for a Clasification of Periodontal Diseases and Conditions, en 1999, cuando se aprueba la clasificación que se expone en este trabajo. En él, se hace una revisión global de los diferentes cuadros de las enfermedades periodontales. Posteriormente, se propone el empleo de antibioterapia de utilización sistémica como la amoxicilina, amoxicilina-clavulánico y metronidazol como primera opción de tratamiento coadyuvante de estas enfermedades.

The association between the IL-1 polymorphism and chronic periodontal disease: A literature review (II)

El propósito de esta revisión es describir el papel que desempeña el polimorfismo de los genes de la IL-1 en la aparición y evolución de la enfermedad periodontal. A lo largo de ella, se observa una falta de homogeneidad en los resultados obtenidos por los diferentes autores. Incluso los estudios más recientes tienden a desestimar la relación que pudiera existir entre la presencia de polimorfismos en los genes de la IL-1 y la enfermedad periodontal. De igual manera, la presencia de estos polimorfismos parece no influir en el fracaso implantario ni en las técnicas de regeneración tisular guiada.

The aim of this bibliographic review is describing the role of the polymorphism in IL-1 genes in the appearance and evolution of the periodontal disease. Through the review, it's observed a lack of homogeneity in the results obtained by different authors. Despite of this, more recent articles tend to have a low opinion of the relationship that could exist between the presence of polymorphism in IL-1 genes and periodontal disease. In the same way, the presence of theses polymorphisms seems not to influence in the implant failure nor in the guided tissue regeneration techniques.

Tooth to implant splinting

Se presenta una revisión sobre la posibilidad terapéutica del empleo de la ferulización entre dientes e implantes. Tras una breve introducción, se establece una serie de diferencias biomecánicas entre dos estructuras tan dispares como son el diente y el implante. Posteriormente, se exponen una serie de puntos que los distintos autores han achacado a este sistema protésico para luego describir los distintos diseños que se pueden aplicar (rígidos y semirrígidos). A continuación, se efectúa una revisión bibliográfica acerca de la evolución a corto y largo plazo de estos sistemas. Por último, se elabora una relación de ventajas, desventajas e indicaciones de la ferulización diente-implante, para terminar estableciendo unas conclusiones.

A review about the tooth-implant splinting therapeutic option is presented. After a brief introduction, it begins with some biomechanical aspects. Then, some critical points about this kind of frames are shown in order to, afterwards, describe de different design that can be applied. A short and long-term evolution of these systems is also made and, finally, we can see some advantages, disadvantages and indications for the splinting option, to finish establishing some conclusions.
