Late hematoma in breast implant surgery is a rare complication. It can occur several years after surgery and most of the times the etiology remains unexplained.

We report a hemorrhagic complication 5 years after aesthetic breast augmentation with silicone prosthesis. Surgical exploration revealed a dark brown fluid collection between the periprosthetic capsule and the implant and signs of bleeding in the muscular plane. The pathological examination reports a fibrous capsule without vasculitis, with a trace of focal hemorrhage and some benign inflammatory cells.
