Cannulation of a central venous route is not free from complications, one of which is the hydrothorax caused by instillation of substances. We present the case of a patient that developed hydrothorax with chemotherapeutic content. The mechanism of action of this hydrothorax was caused by the dissection of both pleural leaves due to the advancement of the tip of the catheter that was placed between the two leaves. We suggest the need for a more comprehensive monitoring after cannulation of central venous catheters.
