Axillary lymph node dissection remains an integral part of surgical treatment in patients with invasive breast cancer and axillary lymph node metastases. Chylous leakage after axillary lymph node dissection is infrequent. We report a case patient with an advanced local breast cancer who was treated with neoadjuvant chemotherapy and surgery. She had a postoperative chylous leakage what was successfully treated conservatively in the postoperative course with somatostatin and a fat reduced diet with medium chain triglyceride. Chylous leakage an infrequent complication after axillary lymph node dissection, which has a conservative management in most published studies.
