Adipose stem cells (ASC) play now a crucial role in reparative or regenerative therapeutic strategies. They are mesenchymal multipotent cells that can be easily obtained from adipose tissue following a liposuction procedure. After an enzymatic digestion and centrifugation step, the stromal vascular fraction (SVF) is obtained in the pellet. ASC can be isolated by subsequent cell culture passages, as these cells are plastic adherent. Over the last years, there are several aspects that have become ASC in an increasing option in cell therapy approaches: the safety demonstrated in clinical trials; the higher amount of cells that can be obtained from adipose tissue compared to bone marrow in a much less invasive and morbid procedure; the possibility of an alogenic application; the immunoregulatory capacity throw molecular mediators secretion or cell-cell direct interaction; and finally, the strong trophic activity that create a suitable microenviroment for activate repair processes in the host tissue. Also in this work, cell doses, routes of administration and indications are outlined for both ASC and SVF applications. In the coming years, ongoing clinical trials in a large variety of diseases will elucidate the huge potential of ASC as well as their mechanisms of action.
