Endoscopic approaches to the anterior skull base have grown very significantly in recent decades and continue expanding parallel to the emergence of new instruments and equipment. Important factors that have contributed to this are the ability to access difficult areas with a direct route and associated decreased morbidity with similar surgical outcomes. But they also have disadvantages and limitations, as they require anatomical knowledge and the use of specific material in a surgical field limited by space, vital anatomical structures and two-dimensional view. The correct surgical planning and patient selection are critical elements. This will allow us to determine if this type of approach is possible, to know the location of the lesion, the entry point to the skull and the corridor through which the entry point will be achieved. Endoscopic approaches to the anterior skull base, as long as correctly indicated and planned, are safe and effective procedures.
