Introduction: Neuropatic pain in cancer patients is a difficult problem which represents a difficult challenge, with good answer to the antiepileptic drugs. Diverse studies exist that confirm the good results associated to the use of topiramate in cancer patients with neuropatic pain. The data demonstrate that antiepileptic drugs can differ from substantially in their effects on weight. Approximately a fifth of patients treated with topiramate experience anorexia and/or weight loss. Clinical cases: Three oncologic patients in the reduction weight associated to the topiramate use was lived like a physical and psychic negative effect. Conclusions: Topiramate is an effective drug in the treatment of the pain neuropatic. The reduction in weight that is from its use can be beneficial in patients with important overweight. This effect could be a contraindication relative to its use in patients with anorexic clinic, severe desnutrition and cancer patients. In these last ones, the appearance of this constitutional syndrome can entail an important increase of episodes of anxiety in the family and the own patient, as well as an excess of medical tests to discard a tumorlike progression.
