At present, the lingual approach is a very good choice for the orthodontic treatment of adult patients. This technique not only allows for the solution of any kind of malocclusion, but also satisfies the aesthetic requirements of the patients. It is not the purpose of this article to explain deeply the differences among several approaches to this technique, but to describe briefly the steps in planning and developing of a treatment of this type. The precise placement of brackets , an efficient bonding protocol and the design of the correct individual arch form are the most important factors besides the training of the operator to reach succes. The bad experience gathered twenty years ago, when many professionals failed with this technique due to the lack of proper training, should not be forgotten. It requires a lot of time to learn this technique, since its biomechanics works differently than that of the vestibular technique, and knowledge should be gained little by little. Training is one of the key issues to get a proper finish of the cases with this technique.
