The elaboration of Care Agenda as a methodological, assistential and educational tool for cares in Cardiovascular Ambulatorial Nursing, is a process that accompanies the proceedings of planning, investigation and validation, so providing new knowledge. Research was realized for a Master' Dissertation onclients with mitral valve implant, at the unit of Cardiology in Hospital Federal dos Servidores do Estado in Rio de Janeiro, Brazil. Objectives: To ease the learning of clients for habitual cares, after discharge; To identify the daily living situations of clients who had learned cares for the solution of the problems. Methodology: descriptive, exploratory, with qualitative and quantitative approach, which allowed us to describe and make comments about impediments and the needs related by clients with mitral valve implant after discharge, in the construction of guide of care. The sample was 48 clients aged 20 to 59 years-old. Data collection was done with research techniques of participant observation (PO), the interview with semi-structured questions and an interview with free or non-structured questions and an outline the Care Agenda. Theoretical and practical incorporated knowledge and knowledge of the client were refined with the reapplication of new knowledge according to the needs of the clients. The research was submitted to Committee of Ethics in Research of the Institution and the clients signed the Term of Free and Clear Approval. Criterion for inclusion: adults of both genders, healthy, 20 to 59 years-old. Exclusion criteria were clients who did not participate of all phases of elaboration of the instrument. Results: The clients who are temporally impeded by sleep, pain, memory difficultiesand who face challenges in their own reality want themselves to construct their own autonomy, thus arousing interest to support a new situation in the health and disease process. The interest of clients in learning of cares is linked to the positive effects which daily well being can provide. Conclusions: The adaptations of practices of cares in the daily lives of clients should attend to the integrality of assisted clients and should involve receptivity, permanent educational support and positive results contributing to adhesion of medical and behavioral therapeutics in obtaining a healthy life.
