A prospective, randomized study with a six-month follow-up.]]> http://scielo.isciii.es/scielo.php?script=sci_arttext&pid=S0213-12852025000600109&lng=en&nrm=iso&tlng=en RESUMEN Introducción: La preservación del hueso crestal es una de las variables más importantes para el éxito de los implantes dentales, ya que influye directamente en su supervivencia y en los resultados estéticos del tratamiento. Objetivos: Se presenta un estudio con el objetivo principal de comparar la pérdida ósea marginal radiográfica en implantes unitarios colocados a nivel crestal y a nivel mucoso, en premolares y molares en maxilar y mandíbula, durante un seguimiento de seis meses. El objetivo secundario fue evaluar los resultados del análisis de frecuencia de resonancia en la estabilidad primaria y su evolución hasta la integración completa del implante. Maerial y métodos: Selección de 32 pacientes con una ausencia dentaria para reposición con implante en premolar o molar maxilar o mandibular. Se colocaron 16 implantes a nivel crestal sin cuello pulido para comparar con 16 implantes colocados con cuello pulido en la porción transmucosa. La variable principal del estudio ha sido comparar la pérdida ósea radiológica entre ambos grupos. La segunda variable analizada ha sido el valor del análisis de frecuencia de resonancia (AFR) en el momento de la cirugía, segunda fase y 6 meses. Resultados: Los implantes yuxtaoseos presentan una mayor pérdida ósea, de 0.22mm mientras que los implantes con cuello pulido fueron menor, de 0.04mm aunque este resultado no es estadísticamente significativo en los 6 meses de seguimiento. Conclusiones: No existieron diferencias significativas entre la perdida ósea de implantes yuxtaóseos respecto a los situados a nivel de tejido.<hr/>ABSTRACT Introducción: Crestal bone preservation is one of the most important variables for the success of dental implants, as it directly influences their survival and the aesthetic results of the treatment. Objectives: This study presents the primary objective of comparing radiographic marginal bone loss in single-unit implants placed at the crestal and mucosal levels in premolars and molars in the maxilla and mandible during a six-month follow-up. The secondary objective was to evaluate the results of resonance frequency analysis on primary stability and its progression until full implant integration. Materials and methods: Thirty-two patients with missing teeth were selected for implant replacement in a maxillary or mandibular premolar or molar. Sixteen crestal implants without a polished neck were placed, compared with 16 implants with a polished neck in the transmucosal portion. The primary endpoint of the study was to compare radiographic bone loss between the two groups. The second variable analyzed was the resonance frequency analysis (RFA) value at the time of surgery, the second phase, and at 6 months. Results: Juxta-osseous implants showed greater bone loss, 0.22 mm, while polished-neck implants showed less, 0.04 mm, although this result was not statistically significant at the 6-month follow-up. Conclusions: There were no significant differences in bone loss between juxta-osseous implants and those placed at the tissue level. <![CDATA[The clinical effectiveness of beta-tricalcium phosphate in dental implant treatment.]]> http://scielo.isciii.es/scielo.php?script=sci_arttext&pid=S0213-12852025000600118&lng=en&nrm=iso&tlng=en RESUMEN Introducción: El objetivo del presente estudio era mostrar los resultados del tratamiento con implantes dentales insertados en diversas situaciones clínicas utilizando el biomaterial betafosfato tricálcico. Material y métodos: 63 pacientes con pérdidas dentales fueron tratados con 140 implantes Galimplant®. En todos los casos, como biomaterial se utilizó el betafosfato tricálcico Osteoblast®. Los implantes fueron cargados después de un periodo de cicatrización de 6 meses. Resultados: Los hallazgos clínicos indican una supervivencia y éxito de los implantes del 95%. 7 implantes se perdieron durante el seguimiento clínico medio de 87,4 meses. En 25 pacientes (39,7%) se realizó la elevación lateral del seno maxilar. En 10 pacientes (15,8%) la elevación sinusal fué transalveolar. En 28 pacientes (44,4%) se utilizó el β-TCP como material de relleno durante técnicas de preservación alveolar. Se realizaron un total de 74 prótesis (100%), 48 puentes fijos (64,9%) y 26 coronas unitarias (35,1%). Conclusiones: Este estudio indica que el beta-fosfato tricálcico puede ser utilizado con éxito como material de regeneración ósea en el tratamiento con implantes dentales.<hr/>ABSTRACT Introduction: The aim of this study was to report the outcome of treatment with dental implants inserted in different clinical situations with use of beta-tricalcium phosphate. Material and methods: 63 patients with tooth loss were treated with 140 Galimplant ® implants. In all cases, beta-tricalcium phosphate Ostoeblast ® were used. Implants were loaded after a healing free-loading period of 6 months. Results: Clinical results indicate a survival and success rate of implants of 95%. 7 implants were lost during the follow-up period of 87,4 months. In 25 patients (39.7%), lateral sinus elevation was performed. In 10 patients (15.8%), transalveolar sinus elevation was performed. In 28 patients (44.4%), β-TCP was used as a filling material during alveolar preservation techniques. A total of 74 prostheses (100%), 48 fixed bridges (64.9%), and 26 single crowns (35.1%) were performed. Conclusions: This study indicate that beta-tricalcium phosphate can be used with success as biomaterial of bone regeneration in treatment with dental implants. <![CDATA[Ten-year clinical follow-up of treatment with crowns on single implants: success, marginal bone loss, biological and prosthodontic complications]]> http://scielo.isciii.es/scielo.php?script=sci_arttext&pid=S0213-12852025000600126&lng=en&nrm=iso&tlng=en RESUMEN Introducción: La implantología oral representa actualmente la mejor opción de tratamiento para la rehabilitación de los pacientes con pérdidas dentales. El presente estudio clínico muestra la evaluación de los pacientes tratados con coronas unitarias mediante la carga precoz de los implantes unitarios. Material y métodos: 61 pacientes con pérdidas dentales unitarias fueron tratados con implantes con superficie arenada y grabada IPX Galimplant ®. Los implantes fueron cargados funcionalmente tras un periodo de tiempo de 6 semanas en la mandíbula y 8 semanas en el maxilar superior. Los hallazgos clínicos (implantológicos y prostodóncicos) se han seguido durante 10 años. Resultados: 109 implantes fueron insertados en ambos maxilares (67 en el maxilar superior y 42 en la mandíbula) para su rehabilitación prostodóncica con coronas implantosoportadas. 31 implantes fueron insertados en el sector anterior y 78 implantes en el sector posterior. Después de 10 años de seguimiento clínico, los resultados indican una supervivencia y éxito de los implantes del 96,3%. Durante el periodo de cicatrización, se perdieron 2 implantes por movilidad, mientras que 2 implantes se perdieron por periimplantitis. 11 implantes presentaron mucositis. 5 implantes presentaron periimplantitis. Complicaciones prostodóncicas se presentaron en 8 coronas. Conclusiones: Los hallazgos clínicos del presente estudio indican que la rehabilitación prostodóncica con coronas unitarias mediante la inserción de implantes de conexión interna y superficie tratada, representa una terapéutica odontológica con éxito.<hr/>ABSTRACT Introduction: Implant dentistry constitutes the best modality in the prosthodontic treatment of patients with tooth loss. This clinical study reports the evaluation of patients treated with single crowns by early loading of single implants. Material and Methods: 61 patients with single-tooth loss were treated with IPX Galimplant® sandblasted and acid-etched surface implants. Implants were loaded after a healing period of 6 weeks (mandible) and 8 weeks (maxilla). Clinical findings (implants and prosthodontics) were followed during 10 years. Results: 109 implants were inserted (67 maxillary and 42 mandibular) for prosthodontic rehabilitation with single-tooth crowns. 31 implants were inserted in anterior sites and 78 in posterior sites. After 10-year follow-up, clinical results indicate a survival and success rate of implants of 96,3%. 2 implants were lost during the healing period by mobility, while 2 implants were lost by peri-implantitits. Eleven implants showed mucositis and 5 implants, reported peri-implantitis. Technical complications showed 8 cases. Conclusions: Clinical results of this study indicate that single crowns supported by internal connection and treated surface implants can be a successful dental treatment. <![CDATA[Technical complications on edentulous geriatric patients treated with fixed rehabilitation with implants. A 7-year study]]> http://scielo.isciii.es/scielo.php?script=sci_arttext&pid=S0213-12852025000600135&lng=en&nrm=iso&tlng=en RESUMEN Introducción: El objetivo del presente estudio era mostrar las complicaciones prostodóncicas en el tratamiento con rehabilitación fija mediante implantes dentales en pacientes geriátricos edéntulos totales. Material y Métodos: 17 pacientes geriátricos edéntulos fueron tratados con 191 implantes con superficie arenada y grabada para la rehabilitación total fija. Los implantes fueron cargados inmediatamente en el mismo día de la cirugía. El seguimiento clínico medio fué de 82,5 ±32,1 meses. Resultados: Los hallazgos clínicos indican una supervivencia y éxito de los implantes del 100%. Los pacientes fueron rehabilitados con 34 prótesis totales atornilladas (30 prótesis híbridas y 4 rehabilitaciones metalcerámica). En 4 pacientes (23,5%), se presentaron complicaciones prostodóncicas en 6 prótesis (17,6%), relacionadas fundamentalmente con fracturas de la resina en las prótesis totales híbridas, aflojamiento de tornillo protésico y fractura de la cerámica. El éxito prostodóncico fué del 82,4%, aunque no hubo necesidad de sustituir ninguna prótesis. Conclusiones: Este estudio indica que la rehabilitación fija mediante implantes dentales en pacientes edéntulos totales constituye una terapéutica con éxito, aunque pueden presentarse complicaciones prostodóncicas.<hr/>ABSTRACT Introduction: The aim of this study was to report the outcome of marginal bone loss in the treatment of fully edentulous patients with fixed prostheses supported with dental implants. Material and Methods: 17 edentulous patients were treated with 191 sandblasted and acid surface implants for fixed maxilla/mandible rehabilitation. Implants were immediately loaded in the same day of the surgery. The mean follow-up was 82.5 ±32.1 months. Results: Clinical results indicate a survival and success rate of implants of 100%. The patients were restored with 34 screw fixed rehabilitations (30 hybrid prostheses and 4 metalceramic restorations). In 4 patients (23.5%), prosthetic complications occurred in 6 prostheses (17.6%), mainly related to resin fractures in hybrid total prostheses, prosthetic screw loosening, and ceramic fracture. Prosthetic success rate was 82.4%, although no prosthesis replacement was required. Conclusions: This study indicates that fixed rehabilitation using dental implants in completely edentulous patients is a successful therapy, although prosthodontic complications may occur. <![CDATA[Preliminary study of the angle and emergence profile of implant-supported crowns. A preliminary study.]]> http://scielo.isciii.es/scielo.php?script=sci_arttext&pid=S0213-12852025000600142&lng=en&nrm=iso&tlng=en RESUMEN Objetivo: Evaluar si el ángulo y el perfil de emergencia de las coronas sobre implantes colocadas en el Hospital Odontológico de la Universidad de Barcelona se ajustan a las recomendaciones vigentes basadas en la evidencia. Material y métodos: Se analizaron 164 radiografías periapicales. El ángulo de emergencia se midió en las superficies mesial y distal y se clasificó como ≤ 30° o &gt; 30°. El perfil de emergencia se categorizó como cóncavo, recto o convexo. Se registraron el tipo de implante (a nivel óseo vs a nivel tisular) y la conexión corona-implante (directa vs mediada por pilar). El investigador principal fue calibrado (CCI intraobservador &gt; 0,90). Se emplearon estadísticos descriptivos y pruebas χ² para calcular frecuencias y evaluar asociaciones. Resultados: Se observó un ángulo de emergencia ideal (≤ 30°) en el 53,8 % de las superficies mesiales y en el 24,4 % de las distales. Los implantes a nivel óseo y las conexiones mediadas por pilar presentaron ángulos de emergencia significativamente más favorables (p &lt; 0,05). En conjunto, el 80 % de las coronas mostraron un perfil de emergencia cóncavo, con mejores resultados en las coronas conectadas mediante pilar que en las de conexión directa (p &lt; 0,05). Conclusiones: Aproximadamente la mitad de las superficies mesiales y una cuarta parte de las distales alcanzaron el ángulo de emergencia recomendado, mientras que cuatro de cada cinco coronas presentaron un perfil de emergencia cóncavo. Los implantes a nivel óseo y, en particular, el uso de pilares transmucosos se asociaron con diseños restauradores más favorables.<hr/>ABSTRACT Objective: To evaluate whether the emergence angle (EA) and emergence profile (EP) of implant-supported crowns placed at the University of Barcelona Dental Hospital conform to current evidence-based recommendations. Material and Methods: A total of 164 periapical radiographs were analysed. EA was measured on the mesial and distal surfaces and classified as ≤30° or &gt;30°. EP was categorised as concave, straight, or convex. The type of implant platform (bone-level vs tissue-level) and the crown–implant connection (direct vs abutment-mediated) were recorded. The principal investigator was calibrated (intra-observer ICC &gt; 0.90). Descriptive statistics and χ² tests were used to calculate frequencies and assess associations. Results: An ideal EA (≤30°) was observed in 53.8 % of mesial and 24.4 % of distal surfaces. Bone-level implants and abutment-mediated connections showed significantly more favourable EAs (p &lt; 0.05). Overall, 80 % of crowns exhibited a concave EP, with better outcomes for abutment-connected crowns than for direct connections (p &lt; 0.05). Conclusions: Approximately half of the mesial and one quarter of the distal surfaces achieved the recommended EA, while four out of five crowns displayed a concave EP. Bone-level implants and, in particular, the use of transmucosal abutments were associated with more favourable restorative designs. <![CDATA[Alveolar preservation with platelet-rich fibrin and bovine xenograft. Dimensional changes.]]> http://scielo.isciii.es/scielo.php?script=sci_arttext&pid=S0213-12852025000600147&lng=en&nrm=iso&tlng=en RESUMEN Introducción: El objetivo del presente estudio era mostrar los resultados clínicos y radiológicos del tratamiento con implantes dentales en la preservación alveolar con PRF y xenoinjerto. Material y métodos: 48 pacientes fueron tratados con extracciones dentales de molares inferiores sin posibilidad de tratamiento conservador y preservación alveolar con PRF y xenoinjerto bovino Lumina-Bone Grosso ®. Los cambios dimensionales pre y postoperatorios (a los 6 meses) fueron valorados por CBCT. Resultados: La preservación alveolar fué realizada con un éxito del 100%. La distancia vestíbulo-lingual media antes de la extracción fué de 10,6 mm, mientras que esta distancia se redujo a 8,8 mm. Siendo la diferencia media de 1,8 mm ±5,8mm lo que supone una variación media del 17,4%. Los molares estudiados (36, 37, 46 y 47) mostraron patrones de reducción comparables, sin diferencias significativas según la ubicación. Conclusiones: Este estudio indica que el PRP con xenoinjerto bovino puede ser utilizado con éxito en la preservación alveolar con cambios dimensionales favorables.<hr/>ABSTRACT Introduction: The objective of this study was to demonstrate the clinical and radiological results of dental implant treatment for alveolar preservation with PRF and xenograft. Material and Methods: 48 patients underwent extractions of mandibular molars without the possibility of conservative treatment and alveolar preservation with PRF and the Lumina-Bone Grosso® bovine xenograft. Preoperative and postoperative dimensional changes (at 6 months) were assessed by CBCT. Results: Alveolar preservation was achieved with 100% success. The mean buccolingual distance before extraction was 10,6 mm, while this distance was reduced to 8,8 mm. The mean difference was 1,8 mm ± 5,8 mm, representing a mean variation of 17,4%. The molars studied (36, 37, 46, and 47) showed comparable reduction patterns, with no significant differences by location. Conclusions: This study indicates that bovine xenograft PRP can be successfully used in alveolar preservation with favorable dimensional changes. <![CDATA[Evaluation of the extracellular matrix (ECM) of porcine small intestinal submucosa (SID) as a biomaterial for the growth of human fibroblasts.]]> http://scielo.isciii.es/scielo.php?script=sci_arttext&pid=S0213-12852025000600155&lng=en&nrm=iso&tlng=en RESUMEN Introducción: la matriz extracelular de la submucosa del intestino delgado (MEC-SID) de porcino ha sido usada con éxito como soporte para regenerar diversos tejidos debido a su composición en colágenos, proteoglicanos, glicosaminoglicanos y factores de crecimiento. Materiales y métodos: en este estudio se caracterizó como biomaterial la MEC-SID a nivel funcional, ultraestructural y se evaluó su capacidad de proporcionar un ambiente adecuado para la adhesión, crecimiento y multiplicación de células humanas. Resultados: se obtuvo un biomaterial completamente descelularizado, con un grosor de ±23µm y la presencia de fibras colágenas de ±1,8µm para la presentación de 1 capa, una porosidad de ±55,4% y una capacidad de retención de líquidos de 386%. El biomaterial mostró un módulo de elasticidad dependiente de la temperatura y del número de capas con las cuales se fabricaba con un rango entre 27,6MPa y 96,8MPa. Se identifico que el biomaterial presento una estabilidad en condiciones de cultivo superior a las cuatro semanas. El biomaterial presento un valor para el ángulo de contacto de ± 30º, la citotoxicidad del biomaterial fue baja con valores inferiores al 10%. Se evaluó la interacción que las células establecieron con el biomaterial mediante microcopia electrónica de barrido, la cual permitió la identificación de proyecciones de citoplasma al igual que la generación de matriz extracelular propia sobre el andamio de MEC-SID. Conclusiones: los resultados obtenidos soportan la hipótesis que este biomaterial podría ser empleado como alternativa para la regeneración de tejidos cutáneos afectados por heridas crónicas y agudas.<hr/>ABSTRACT Introduction: the porcine small intestinal submucosal extracellular matrix (SI-SEM) has been successfully used as a scaffold for regenerating tissues due to its composition of collagens, proteoglycans, glycosaminoglycans, and growth factors. Materials and Methods: in this study, the SI-SEM as a biomaterial was characterized at the functional and ultrastructural levels, and its ability to provide a suitable environment for the adhesion, growth, and multiplication of human cells was evaluated. Results: a completely decellularized biomaterial was obtained, with a thickness of ±23 µm and the presence of collagen fibers of ±1.8 µm for the single-layer presentation, a porosity of ±55.4%, and a fluid retention capacity of 386%. The biomaterial showed an elastic modulus that depended on temperature and the number of layers with which it was manufactured, ranging from 27.6 MPa to 96.8 MPa. The biomaterial was found to be stable under culture conditions for over four weeks. The biomaterial exhibited a contact angle of ±30°, and its cytotoxicity was low, with values below 10%. The interaction of the prepared cells with the biomaterial was evaluated by scanning electron microscopy, which allowed the identification of cytoplasmic projections as well as the generation of extracellular matrix on the SI-SEM scaffold. Conclusions: the results obtained support the hypothesis that this biomaterial could be used as an alternative for the regeneration of skin tissues affected by chronic and acute wounds.
