Frente al predominio de las tradiciones filosóficas centroeuropeas en el desarrollo de la psiquiatría y la psicología fenomenológico-existenciales a mediados del siglo XX, la obra de Ortega representa una contribución singular del ámbito hispánico, aún escasamente explorada en este terreno. Su concepción de la vida humana como “realidad radical”, su perspectivismo y la célebre fórmula “yo soy yo y mi circunstancia” ofrecen categorías filosóficas que permiten comprender al ser humano no como una entidad fija o natural, ni reducible a procesos biológicos o mentales individuales, sino como un proyecto vital abierto, situado e históricamente configurado. Estas nociones proporcionan herramientas conceptuales valiosas para abordar problemas clínicos contemporáneos, tales como el reduccionismo biomédico dominante, el dualismo implícito en la psicología cognitivo-conductual y la necesidad de una antropología que contemple el mundo vivido, la corporalidad, la intersubjetividad y el papel de la circunstancia social y de la narración en la configuración del yo. Se sostiene que, aunque Ortega no elaboró una teoría clínica explícita, sus planteamientos antropológico-filosóficos constituyen un marco conceptual fecundo para repensar la clínica y la psicoterapia desde una orientación fenomenológico-existencial, más atenta al entramado vital, histórico y cultural en el que se inscribe el sufrimiento humano.<hr/>Abstract This paper examines the contributions and potential of José Ortega y Gasset’s philosophy as a framework for conceptualizing a phenomenological–existential clinic and psychotherapy centered on human life in its concreteness. In contrast to the predominance of Central European philosophical traditions in the development of phenomenological–existential psychiatry and psychology during the mid-twentieth century, Ortega’s work represents a distinctive contribution from the Hispanic intellectual sphere, still scarcely explored in this field. His conception of human life as a “radical reality,” his perspectivism, and the celebrated formula “I am I and my circumstance” provide philosophical categories that make it possible to understand the human being not as a fixed or natural entity, nor as reducible to individual biological or mental processes, but rather as an open, situated, and historically constituted life-project. These notions offer valuable conceptual tools for addressing contemporary clinical issues such as dominant biomedical reductionism, the implicit dualism of cognitive-behavioral psychology, and the need for an anthropology that takes into account the lived world, corporeality, intersubjectivity, and the role of social circumstance and narration in the constitution of the self. It is argued that, although Ortega did not develop an explicit clinical theory, his anthropological and philosophical insights constitute a fertile conceptual framework for rethinking clinical practice and psychotherapy from a phenomenological–existential orientation— one more attuned to the vital, historical, and cultural fabric in which human suffering is embedded. <![CDATA[Definir la salud mental en un mundo diverso: Hacia una atención y cuidados culturalmente sensibles]]> http://scielo.isciii.es/scielo.php?script=sci_arttext&pid=S0211-57352025000200059&lng=es&nrm=iso&tlng=es Resumen El artículo explora cómo podemos definir el concepto de salud mental en la sociedad actual, teniendo en cuenta la infinidad de contextos culturales que existen hoy día. A nivel mundial, la Organización Mundial de la Salud y el movimiento de Salud Mental Global han planteado diferentes maneras de enfocar y comprender la salud mental de las personas. El artículo busca resaltar y visibilizar las diferencias culturales en la percepción y tratamiento del sufrimiento psíquico. También aborda la influencia del estigma y la falta de acceso a servicios adaptados culturalmente para poder comprender mejor qué es la salud mental. Se enfatiza la necesidad de un enfoque transcultural que equilibre lo biomédico con factores socioculturales, promoviendo prácticas inclusivas y culturalmente sensibles. La salud mental debe ser englobada dentro de su contexto social, evitando reduccionismos y favoreciendo una integración de conocimientos tradicionales y científicos para un cuidado más equitativo y eficaz.<hr/>Abstract The article explores how we can define the concept of mental health in today's society, considering the vast array of cultural contexts that exist. Globally, the World Health Organization and the Global Mental Health movement have proposed different ways to approach and understand people's mental health. This paper aims to highlight and make visible cultural differences in the perception and treatment of psychological suffering. It also addresses the influence of stigma and the lack of access to culturally adapted services to better understand what mental health is. The need for a transcultural approach that balances biomedical aspects with sociocultural factors is emphasized, promoting inclusive and culturally sensitive practices. Mental health should be understood within its social context, avoiding reductionism and encouraging the integration of traditional and scientific knowledge for more equitable and effective care. <![CDATA[Florencio Villa Landa (1912-1992): Un psiquiatra español en la Unión Soviética y en la revolución cubana]]> http://scielo.isciii.es/scielo.php?script=sci_arttext&pid=S0211-57352025000200077&lng=es&nrm=iso&tlng=es Resumen Presentamos la biografía del psiquiatra pacense Florencio Villa Landa, uno de los escasos médicos españoles que se exiliaron a la Unión Soviética durante nuestra Guerra Civil. En el intermedio de dos estancias en dicho país, nuestro protagonista estuvo exiliado en México y en Cuba, contribuyendo notablemente en este último país al desarrollo de la psiquiatría tras el triunfo de la revolución. Regresó definitivamente a España a final de la década de 1970, una vez instaurada la democracia.<hr/>Abstract We present the biography of the psychiatrist from Badajoz (Spain) Florencio Villa Landa, one of the few Spanish doctors who went into exile in the Soviet Union during our Civil War. In the middle of two stays in that country, our protagonist completed his exile in Mexico and Cuba. In the latter country, he remarkably contributed to the development of psychiatric services after the triumph of the revolution. He finally returned to Spain at the end of the 70s, once Spanish democracy was established. <![CDATA[Organismo. Dentro del conjunto de corrientes psicodinámicas y psicoanalíticas que convivieron en este período, este estudio se centra en la recepción de la obra de Jacques Lacan y en el papel que tuvieron los psicoanalistas argentinos exiliados de la dictadura militar. Como forma de adentrarnos en este contexto de efervescencia política y de diversidad formativa, este trabajo incluye fragmentos de entrevistas realizadas a algunos de los jóvenes que participaron en este contexto, en las que se brinda una lectura más cercana y personal sobre cómo fueron las trayectorias de estudio y acceso al psicoanálisis en los años 70, principalmente en Madrid, y el cambio que se produjo a partir de la llegada del Campo Freudiano en los años 80.<hr/>Abstract This paper analyses the political context and intellectual changes that made possible the return of critical psychoanalysis in the late Franco era and the democratic Transition. Among the various psychodynamic and psychoanalytic currents that coexisted during this period, this study focuses on the reception of Jacques Lacan's work and the role played by Argentinian psychoanalysts exiled from the military dictatorship. As a way of exploring this context of political effervescence and educational diversity, this work includes extracts from interviews with some of the young people who participated in this context, which provide a closer and more personal insight into their study trajectories and access to psychoanalysis in the 1970s, mainly in Madrid, and the change that took place with the arrival of the Freudian Field in the 1980s. <![CDATA[Una mirada psicoanalítica sobre la práctica en recursos de rehabilitación psicosocial en personas con psicosis]]> http://scielo.isciii.es/scielo.php?script=sci_arttext&pid=S0211-57352025000200149&lng=es&nrm=iso&tlng=es Resumen El presente texto describe y profundiza en la práctica de los dispositivos de Rehabilitación Psicosocial para personas con enfermedad mental articulándola con la teoría psicoanalítica. La teoría psicoanalítica permite pensar y dotar de sentido una práctica institucional que se muestra simple a primera vista, quizá porque su trabajo se realiza en la cotidianidad de los recursos y de la vida de las personas atendidas. Qué se hace y para qué es la pregunta que trata de desplegarse. A lo largo del texto se comentan aspectos a salvaguardar para garantizar una buena práctica institucional que tenga efectos beneficiosos en los sujetos. Se hace especial hincapié en la importancia de evitar la normalización y homogeneización de los sujetos y de favorecer un ambiente vital que colonice la pulsión de muerte. Por último, se describen los retos y los obstáculos que han de orillarse.<hr/>Abstract This article describes and examines the practice of psychosocial rehabilitation resources for individuals with mental illness from a psychoanalytic approach. Psychoanalysis offers a framework to give depth and meaning to an institutional practice that might appear straightforward at first glance, perhaps because it operates within the everyday lives of both the resources and the individuals in care. What is being done, and for what purpose? This is the question the article attemps to address. Throughout the text, key elements necessary to ensure effective institutional practices that have a positive impact on individuals are discussed. Particular emphasis is placed on avoiding the normalization of people’s lives and behaviors, and fostering a living environment that colonizes the death drive. Lastly, the main challenges and obstacles that must be confronted to sustain this approach are discussed. <![CDATA[Clínica analítica en el margen de la salud mental]]> http://scielo.isciii.es/scielo.php?script=sci_arttext&pid=S0211-57352025000200165&lng=es&nrm=iso&tlng=es Resumen Este artículo explora la consistencia y los efectos de la clínica analítica en el margen de la salud mental aplicada en un museo de arte contemporáneo (MACBA, Barcelona, 2002-2013). Se presentan dos observaciones que provienen de este dispositivo sobre dos adultos en la cincuentena diagnosticados de esquizofrenia, y se aportan unas notas sobre la histórica relación entre poder, producción artística y salud mental. En un anexo, se incluye una conversación con Eva Marxen, la arteterapeuta que dirigió el dispositivo arteterapéutico, contemporáneo al analítico, trabajando con adolescentes en situación de riesgo social.<hr/>Abstract This article explores the consistency and effects of analytical clinical practice on the margins of mental health applied in a contemporary art museum (MACBA, Barcelona, 2002-2013). Two observations coming from this device, concerning two adults in their fifties diagnosed with schizophrenia, are presented and some notes on the historical relationship among power, artistic production, and mental health are provided. In an annex, a conversation with Eva Marxen, the art therapist who led the art therapy program, contemporary to the analytical one, with adolescents in situations of social risk, is included. <![CDATA[Labrarse un lugar <em>éxtimo</em>]]> http://scielo.isciii.es/scielo.php?script=sci_arttext&pid=S0211-57352025000200195&lng=es&nrm=iso&tlng=es Resumen Ilan Pappé, uno de los nuevos historiadores israelíes, profundamente crítico con el proyecto sionista, describe las prisiones como “estructuras permanentes, inmunes al escrutinio internacional y que operan como un mundo independiente” (1). Aunque en las democracias formales la privación de libertad sea consecuencia de un proceso judicial, suspender temporalmente los más elementales derechos humanos tiene consecuencias subjetivas. Si la apuesta es que los sujetos ahí encerrados puedan responder de manera diferente a las dificultades de la vida, es imprescindible, en semejante limbo, introducir una clínica de la escucha que descomplete la mirada panóptica. El psicoanálisis, advertido de la faz oscura de los imperativos institucionales e interesado en la responsabilidad subjetiva, puede ser brújula que oriente en el caso por caso y, desde ese lugar éxtimo, favorecer otra inserción posible del sujeto en el lazo social. Un breve análisis crítico del tratamiento penitenciario, a la luz del discurso analítico, enmarca el recorrido de la autora por diferentes centros penitenciarios a lo largo de treinta y cinco años. Dócil a las palabras de las personas presas, se ha dejado enseñar por ellas; de ello intenta dar cuenta rescatando algunos pasajes de su praxis.<hr/>Abstract Ilan Pappé, one of the new Israeli historians, deeply critical of the Zionist project, describes prisons as “permanent structures, immune to international scrutiny and operating as an independent world” (1). Although in formal democracies the deprivation of liberty is the consequence of a judicial process, temporarily suspending the most elementary human rights is not without subjective consequences. If the bet is that the subjects locked up can respond differently to the difficulties of life, it is essential, in such limbo, to introduce a clinic of listening that decompletes the panoptic gaze. Psychoanalysis, aware of the dark face of institutional imperatives and interested in subjective responsibility, can be a compass that guides in the case by case and, from that place of excellence, might favour another possible insertion of the subject in the social bond. A brief critical analysis of penitentiary treatment, in the light of analytical discourse, frames the author´s journey through different prisons over thirty-five years. Docile to the words of prisoners, she has allowed herself to be taught by them; in this paper she tries to account for this by rescuing some passages of her praxis. <![CDATA[Aportaciones del psicoanálisis a la educación]]> http://scielo.isciii.es/scielo.php?script=sci_arttext&pid=S0211-57352025000200209&lng=es&nrm=iso&tlng=es Resumen La escuela es una institución socializadora y no terapéutica. Así como uno tiene una familia que lo condicionará en múltiples aspectos de su vida, también pasará por una escuela determinada y por un aprendizaje u otro. Este artículo ayudará a reflexionar acerca de las posibles aportaciones del psicoanálisis en el mundo educativo, sobre todo entendiéndolo como una posible teoría que, junto con otras y con otros aspectos, puede aportar una manera de entender lo que se pone en juego en el aprendizaje, en sus dificultades, así como en todo lo que influye en el establecimiento del vínculo educativo. La escuela de hoy debe ser un espacio de construcción donde la subjetividad sea importante. Desde el psicoanálisis podemos estudiar la relación que se establece entre el docente y el discente en el marco de una relación transferencial. La transferencia y el deseo son el motor del hecho psicológico de aprender. También son importantes las relaciones que establecen con otros fuera del grupo familiar, que serán determinantes para su futuro.<hr/>Abstract Schooling is not something external and casual for a person, it is a long experience with a deep and singular meaning that covers an important period in human development. This article helps to reflect on the posible contributions of psychoanalysis in the educational world, understanding it as a possible theory, which, together with others and with other aspects, can provide a way of understanding what is at stake in learning. Today’ school must be a space of construction where subjectivity matters. Psychoanalysis might be useful to study the relationship established between the teacher and the student in the framework of a transferential relationship. Transference and desire are the driving forces of the psychological fact of learning. The relationships established with peers might also be determinant for their future. <![CDATA[Aportaciones de la asistencia pública a la clínica psicoanalítica: psicoterapia, psicopatología y formación]]> http://scielo.isciii.es/scielo.php?script=sci_arttext&pid=S0211-57352025000200227&lng=es&nrm=iso&tlng=es Resumen En su conferencia de Budapest, en 1918, Freud planteó una pregunta que no ha perdido actualidad: ¿por qué caminos debía transitar el psicoanálisis para asegurar su porvenir? Por un lado, advirtió el peligro de que esta práctica quedara confinada a una élite, aislada del saber contemporáneo y de los asuntos de su tiempo. Por otro, insistió en el compromiso social del analista, consistente en brindar su atención sin distinción, tanto al pobre como al rico. Pero Freud no se detuvo ahí. Señaló también que el saber analítico de entonces era todavía imperfecto y que exigía ser ampliado con nuevas aportaciones, así como con una amplia revisión de su técnica. La solución que propuso fue nítida: llevar el psicoanálisis más allá del consultorio privado y hacerlo accesible a quienes no podían costearlo. Algunos de sus discípulos acogieron esa indicación y la llevaron a cabo mediante la creación de las primeras clínicas gratuitas. Aquella decisión marcó un hito en la historia del psicoanálisis. Y desde entonces, no han faltado quienes, fieles a esa orientación, han seguido ejerciendo la clínica analítica en el seno de los servicios públicos de salud mental. De ese esfuerzo, sostenido a lo largo del tiempo, ha surgido un saber psicopatológico de gran valor, nutrido por la experiencia concreta con sujetos muy variados y por el trabajo cotidiano en condiciones muchas veces adversas. Las transformaciones introducidas en el método han permitido, además, atender con eficacia a pacientes con alteraciones graves en el medio institucional, sin renunciar a la orientación analítica. Podemos afirmar, entonces, que el porvenir del psicoanálisis sigue siendo el que Freud vislumbró hace más de un siglo, cuando supo ver que la causa analítica y la causa social no eran dos caminos distintos, sino uno solo.<hr/>Abstract In his 1918 lecture in Budapest, Freud posed a question that remains relevant today: which paths should psychoanalysis take to secure its future? On one hand, he warned of the danger that psychoanalysis might become confined to an elite circle, isolated from contemporary knowledge and the pressing issues of its time. On the other, he emphasized the analyst’s social responsibility —to offer care without distinction, to both the poor and the wealthy. But Freud didn’t stop there. He also pointed out that the analytic knowledge of the time was still incomplete and needed to be expanded through new contributions and a thorough revision of its technique. His proposed solution was clear: to take psychoanalysis beyond the private office and make it accessible to those who could not afford it. Some of his followers embraced this vision and acted on it by establishing the first free clinics. That decision marked a turning point in the history of psychoanalysis. Since then, many have remained true to that direction, practicing analytic work within public mental health services. From this sustained effort, a valuable body of psychopathological knowledge has emerged — one shaped by concrete experience with a wide variety of individuals and forged in often challenging institutional conditions. The methodological innovations developed along the way have made it possible to effectively treat patients with severe disorders within institutional settings, without abandoning an analytic orientation. We can therefore affirm that the future of psychoanalysis remains the one Freud envisioned over a century ago — recognizing that the analytic cause and the social cause are not two separate paths, but one and the same. <![CDATA[El psicoanálisis y su posición respecto de la ciencia]]> http://scielo.isciii.es/scielo.php?script=sci_arttext&pid=S0211-57352025000200245&lng=es&nrm=iso&tlng=es Resumen Partiendo de la diferencia aristotélica entre episteme (saber) y doxa (opinión), el autor muestra cómo el discurso científico fue perdiendo su neutralidad inicial y, alentado por el capitalismo neoliberal, fue transformándose en algunos aspectos en ideología. En este contexto, recuerda las enseñanzas de Lacan a este respecto y analiza la posición del psicoanálisis actual respecto a la ciencia.<hr/>Abstract Starting from the Aristotelian difference between episteme (knowledge) and doxa (opinion), the author shows how the scientific discourse gradually lost its initial neutrality and, encouraged by the neoliberal capitalism prevailing, became in some ways ideology. In this context, the author recalls Lacan's teachings in this regard and analyzes the position of current psychoanalysis regarding science. <![CDATA[El diálogo entre psicoanálisis, feminismo y deseo lesbiano en la obra de Teresa de Lauretis]]> http://scielo.isciii.es/scielo.php?script=sci_arttext&pid=S0211-57352025000200253&lng=es&nrm=iso&tlng=es Resumen El artículo pretende exponer el trabajo de la académica feminista y lesbiana Teresa de Lauretis a partir de las relecturas que realiza del psicoanálisis para aportar una nueva conceptualización del deseo lesbiano, utilizando enfoques provenientes de la teoría feminista, de su concepto de tecnología del género y de una lectura original de los conceptos de perversión, de fetichismo y de pulsión. Su obra pretende cuestionar tanto las visiones heteronormativas dominantes en el psicoanálisis como ciertos discursos feministas que a menudo marginan la experiencia lésbica.<hr/>Abstract This article presents the work of feminist and lesbian scholar Teresa de Lauretis, drawing on her reinterpretations of psychoanalysis to provide a new conceptualization of lesbian desire, utilizing approaches from feminist theory, her concept of technology of gender, and an original interpretation of the concepts of perversion, fetishism, and drive. Her work seeks to challenge both the dominant heteronormative views in psychoanalysis and certain feminist discourses that often marginalize the lesbian experience. <![CDATA[Retos en la integración laboral del profesional de apoyo entre iguales en salud mental]]> http://scielo.isciii.es/scielo.php?script=sci_arttext&pid=S0211-57352025000200271&lng=es&nrm=iso&tlng=es Resumen La figura del profesional de apoyo entre iguales en salud mental representa una innovación para el sistema sociosanitario español. Este modelo introduce la experiencia vivida como una herramienta de apoyo, mediante la incorporación de personas que han atravesado un problema de salud mental y que, tras una formación específica, ofrecen acompañamiento a quienes están en su propio proceso de recuperación. A pesar de sus beneficios potenciales, la implementación de esta figura encuentra resistencias frente al cambio dentro de los modelos actuales de atención. Este texto reflexiona sobre el valor añadido del conocimiento experiencial y la necesidad de integrar a los profesionales de apoyo entre iguales como parte complementaria del equipo interdisciplinar en salud mental.<hr/>Abstract The figure of the peer support professional in mental health represents an innovation for the Spanish healthcare system. This model introduces a lived experience as a support tool, through the incorporation of individuals who have experienced mental health problems and who, following specific training, offer accompaniment to those who are in their own recovery process. Despite its potential benefits, the implementation of this figure encounters resistance to change within current care models. This text reflects on the added value of experiential knowledge and the need to integrate peer support professionals as a complementary part of the interdisciplinary mental health team. <![CDATA[El programa Barraca XXI en el Hospital Psiquiátrico de Conxo]]> http://scielo.isciii.es/scielo.php?script=sci_arttext&pid=S0211-57352025000200287&lng=es&nrm=iso&tlng=es Resumen En abril de 2024, el Hospital Psiquiátrico de Conxo y la Fundación SGAE iniciaron el Programa Barraca XXI, una iniciativa sociocultural dirigida a mejorar la atención de personas con trastorno mental grave mediante talleres creativos de teatro, música y audiovisual. El proyecto buscó fomentar la participación activa de los usuarios, promover la creatividad y transformar la institución desde una perspectiva más humana y comunitaria. A lo largo del proyecto se observaron beneficios terapéuticos significativos, como el fortalecimiento del vínculo grupal, el desarrollo de habilidades sociales y artísticas, y una mayor implicación de los pacientes en su proceso de recuperación. El programa también contribuyó a abrir el hospital a la ciudadanía, reduciendo el estigma asociado a la enfermedad mental. La experiencia ha sido enriquecedora y se plantea su continuidad como parte de un modelo asistencial más inclusivo y centrado en la persona.<hr/>Abstract In April 2024, the Conxo Psychiatric Hospital and the SGAE Foundation (the Spanish Society of Authors and Publishers) launched the Barraca XXI Program, a sociocultural initiative aimed at improving the care for people with severe mental illness by organising workshops involving creative theatre, music, and audio-visual arts. The project sought to encourage users’ active participation, to promote creativity, and to transform the institution from a more humane and community-based perspective. Over the course of the project, significant therapeutic benefits were observed, such as the strengthening of group bonds, the development of artistic and social skills, and more involvement of all of the patients in their recovery process. The program also contributed to opening the hospital to the general public, reducing the stigma associated with mental illness. The experience has been rewarding, and its continuation is being considered as part of a more inclusive and person-centred healthcare model. <![CDATA[De la psiquiatría del fármaco a la salud mental colectiva.
