Shrot-Term Dynamic Psychoterapy: a conceptual and clinical approach

El objetivo central de este artículo es tratar de esclarecer y discutir, desde un posicionamiento teórico y clínico, las principales características definitorias, las indicaciones y contraindicaciones y los matices técnicos básicos de los diferentes tipos de la psicoterapia dinámica breve, una práctica enraizada en el psicoanálisis que viene mostrando una adecuada eficacia, efectividad y eficiencia en algunos trastornos psíquicos.

The aim of this paper is to try to clarify and discuss, from a theorical and clinical point of view, the main definitory features, indications and contraindications as well as basic technical details of the different types of short term psychodynamic psychotherapy. This form of therapy has its own roots in psychoanalysis having showed an adequate efficacy, effectiveness and efficiency in certain mental disorders.

Language disturb in schizophrenia: clinical symptoms and psycholingüistic measures

Estudiamos la coherencia global del discurso en una muestra de sujetos con esquizofrenia y sujetos controles normales, encontrando diferencias significativas entre ambos que se mantienen en el tiempo.

This paper presents the study of global coherence of discourse that shows a sample of schizophrenic patients and matched controls. We found differences statistically significatives within productions of both groups of subjects stables on time.

A critical review of psychoterapeutic treatments for patients with Borderline personality disorder

El autor presenta una revisión sobre los tratamientos psicológicos del trastorno borderline de personalidad. Todas las modalidades psicoterapéuticas revisadas: terapias psicodinámicas, psicoterapia de apoyo, tratamientos conductuales y cognitivos, terapia interpersonal, tratamientos familiares y grupales, subrayan la importancia de cuidar la relación con el paciente y la dificultad en manejar la contratransferencia, el papel activo del terapeuta y la necesidad de crear unos límites claros. La efectividad de los tratamientos está relacionada con la mayor capacidad para controlar los impulsos, la disminución de las conductas suicidas y autoagresivas y el descenso de las hospitalizaciones.

The author provides an overview of the psychological treatments of borderline personality disorder. Among the different psichoterapeutic modalities revised: psychodinamic psychotherapy, supportive psychotherapy, cognitive and behavioural approaches, interpersonal, group andfamily treatments, all approaches stress the importance of careful attention to the relationship with the patient and the difficulty in handling the countertransference, the therapist's active role, the need to set limits. The effectiveness includes the ability of these treatments to obtain a reduction in the lack of self-control, to reduce the life threatening and self-destructive behaviour typical of borderline patients and the decrease the days in hospital.

Research and teaching in Mental Health (II): is there any future for the "Consultant Centered Care" and the clinical point of view?

El trabajo parte de una reflexión epistemológica sobre la investigación en psicoterapia y una descripción de las áreas, métodos y condicionantes para el futuro de la misma. Se describen asimismo algunas consecuencias ilustrativas de los sesgos que han venido mostrándose anteriormente, tanto de la "perspectiva Bambi" como de la "perspectiva Godzilla". Se plantean como ejemplos ilustrativos los siguientes: la santificación del "managed care"; el efecto Utah en el lanzamiento de nuevos neurolépticos y antidepresivos; los resultados negativos de los metaanálisis de una técnica que sigue practicándose y defendiéndose, como es la TEC; los resultados positivos pero poco conocidos de algunas técnicas psicológicas elementales en el cuidado de los pacientes graves y sus familiares así como de la depresión puerperal; los resultados negativos o inciertos de determinadas técnicas "contraculturales", etc. Para terminar, se realizan una serie de propuestas integradoras, tanto para la investigación como para la docencia en salud mental, orientadas por un intento de soslayar los dogmatismos y sectarismos habituales.

Paper begins with an epistemological reflection on psychotherapy research and a description of areas, methods and condition ants for its future. Sorne shunts of Bambi's perspective and Godzilla's perspective are described. Sorne illustrative examples are considered: the sanctification of "managed care", the Utah effect on marketing of new neuroleptics and antidepressants, the meta-analysis negative results about ECT, the positive but almost unknown effects of sorne elementary psychological techniques on severe disorders care and on puerperal depression, the negative results of sorne "contra cultural" techniques... To end, a series of integrative proposals for research and teaching in mental health are developed --with the common objective to avoid the sectarian and dogmatic trends almost habitual in that field.

The scientific dimension of psychoanalysis

Se revisa la relación entre la neurociencia y el psicoanálisis. La neurociencia puede proporcionar una nueva base para el crecimiento futuro del psicoanálisis sin que ello suponga un abandono del conjunto enorme de conocimientos clínicos valiosos que el psicoanálisis ha reunido durante el ultimo siglo.

The relationship between neuroscience and psychoanalysis are examined. Se presenta un caso de morfinomanía de principios de siglo, con el objeto de analizar la larga pugna entre dos saberes disciplinarios -psicopatología y criminología- por la apropiación de un campo entonces naciente en la Argentina, el de las toxicomanías.

A case of morphinism dated in the beginning of the XXth century is used to illustrate the long fight between two disciplines -criminology and psychopathology-over drug addiction, a newly born area of knowledge in Argentina at the time.

Psychopedagogic workshop for panic attack and agoraphobia

Estructura y contenidos de un taller psicopedagógico para pacientes con crisis de ansiedad, con o sin agorafobia y sus familiares. Altamente estructurado, con recurso a diverso material de apoyo, durante 1 mes con 4 sesiones de 1,5 horas, con diferentes actividades y "tareas para casa". Además, se presenta la evolución clínica de 55 participantes, con una significativa reducción en las crisis referidas y niveles de ansiedad generalizada. Junto a ello, un cuestionario de satisfacción recoge una positiva valoración de los participantes.

Structure and contents of a psychopedagogic group for patients with panic attacks, with or without agoraphobia, and their relatives. Highly structured and using diverse helping materials, runs for 1 month in 4 sessions of 1,5 hours, with different activities and "home work". It is also presented the clinical evolution of 55 participants, having observed a significant reduction in the number of reported crisis and level of generalised anxiety. With a satisfaction questionnaire, a positive valorisation is elicited from the participants.
