^les^aLo reactivación de la infección por VHB es una complicación bien documentada del tratamiento citotóxico. Se presenta en el 20-50% de los pacientes con HBsAg positivo sometidos a este tipo de tratamiento, y tiene una morbilidad entre 5-12%¹. No existe un tratamiento estándar y la práctica más habitual es la observación; sin embargo, se han ensayado tratamientos con lamivudina y adefovir dipivoxil con resultados prometedores. La determinación sérica del HBsAg se debe realizar a todos los pacientes con cáncer antes de iniciar la quimioterapia, debiendo administrar lamivudina si existen dotos serológicos de replicación vírica. En este artículo se presenta un caso de hepatitis aguda subfulminante en un paciente con un adenocarcinoma de próstata tratada con adefovir dipivoxil, se revisa el tratamiento y prevención de la reactivación vírica.^len^aReactivation of VHB infection is a well documented complication of cytotoxic treatment. It appears in 20-50% of the patients with positive HBsAg under treatment, and has a mortality ranging from 5 to 12%. There is no standard treatment for the infection, and the current practice is observation. Nevertheless, treatment with lamivudine and adefovir dipivoxil have been tried with promising results. Serum determination of HBsAg must be done to all patients with cancer before initiating chemotherapy, admninistering lamivudine if analytical data indicate the existence of viral replication. A case of subfulminant acute hepatitis of a patient with prostate adenocarcinoma treated with adefovir dipivoxil is presented in this article. Treatment and prevention of viral reactivation are reviewed.
