Capgras syndrome is a well-defined nosological unit, classified into the delusional misidentification syndromes. It has until now provided an empirical ground for the clinical description of delusional interpretation and delusional perception. It has also been used for the neuropsychological modelling of prosopagnosia, identity and familiarity appreciation. A sequential deficit in the sense of familiarity is proposed as the underlying process taking place from estrangement to delusion, which is conceived as a failure of passivity in putting back the affective coherence of experience.
