Abstract

The anatomic knowledge of the breast, specially of its circulation and innervation, allowed a complete manipulation of the breast parenchyma, which made feasible the creation of nipple-areola complex flaps of different kinds for its transposition. In this article, the case of a young female who possessed very divergent nipples in their anatomic position, with complaints of macromastia and bilateral breast ptosis is presented. The surgical steps were the same as described by Pitanguy. Nevertheless, it was added a modification to better transpose the areola to its definitive position without tension, that was a straight-line incision only on the derm of the lateral decorticated margin, which preserved the subdermal vascularization and innervation. It was observed that Pitanguy"s technique associated with a lateral dermal incision on the nipple-areola flap allowed a superomedial transposition in very diverged breasts, because it resulted in a better aesthetical outcome with vascularization and sensibilitymaintenance.
