The World Health Organization recognizes oral carcinoma cuniculatum as a rare and well-diferenciated clinicopathological variant of oral squamous cell carcinoma. The proliferation of stratified squamous epithelium and its infiltration into underlying stroma forms a complex pattern of keratin cores and keratin filled crypts which looks like a "rabbit burrow" or "cuniculi" from the latin. We describe the main characteristics of this entity and we report a case of oral carcinoma cuniculatum of the buccal left mucosa treated with resection and sentinel lymph node biopsy.
