Despite the advances achieved in the clinical and therapeutic knowledge of Periodontics, the clinical management of third molars remains controversial from a periodontal point of view. In the present paper, the features leading to loss of periodontal support associated to impacted lower third molars and the most appropriate therapeutic procedures are analysed based on the available scientific evidence.
