Acute appendicitis is the most common surgical emergency in children. In most cases an adequate medical history and physical examination allows an easy diagnosis. However, sometimes the presence of unusual symptoms and signs as the main manifestation, for example those related to the genitourinary tract, may delay diagnosis leading to the development of complications. We report the case of a 13 year old boy in whom the atypical presentation of acute appendicitis led to inadequate initial treatment and a delay in diagnosis.
