Long term success in implant therapy seems to be c10sely related to optimal plaque control. We have reviewed the recommendations published in the literature concerning professional and home oral hygiene procedures for patients with implant-based prostheses. According to many Authors, these patients should undergo 3-6 monthly regular check-ups, to be established in line with health of periodontal and perimplant tissues. In the large variety of traditional instruments for oral hygiene procedures, the current trend is to avoid metal instruments because they may damage implant surfaces and lead to increasingly plaque adhesion and to alteration of titanium biocompatibility. The preference is given to plastic Teflon (PTFE) instruments which are less aggressive on implant surfaces. In the reported case six implants was expelled from the patient under treatment with lithium based therapy for manic depressive syndrome. We examined all the samples using SEM, and noted macroscopic traces on the neck and body implant surfaces in addition to fine particles of hypothetical plastic originoWe presume these micropartic1es may come from superficial curette flaking made worse, by softening through steam sterilisation. Although plastic Teflon instruments are preferred to others, nowadays they are not the ideal means.
