Introduction: The modification of periodontal structures through orthodontic movement for subsequent implant installation is intimately tied to the periodontal tooth-bone-ligament relationship and to reverse planning in rehabilitations. Objective: To increase the quantity of keratinized mucosa and alveolar bone by slow orthodontic extrusion prior to installing implants and immediate provisionalization. Case report: Slow orthodontic extrusion was performed on the upper central incisors, with extrusive force applied for 1 year. Afted that, the condemned dental elements were extracted, followed by the installation of titanium implants and immediate provisionalization. Conclusion: The slow orthodontic movement of periodontally compromised teeth offers a satisfactory way to modulate the periodontal support/coating before the installation of osseointegrated implants, satisfying aesthetic requirements.
