The prevalence of hip fracture is an important problem of public health because of its frequency, risk and cost. The medical, rehabilitative and social implications have great impact. Predominantly, it affects the elderly and women, and varies in incidence, mortality and hospital stay between regions. We describe the case of an 81 year old woman who had a closed hip fracture, comorbidity and was living with her daughter. Before the fracture, she was walking independently. We focus our attention on four primary diagnoses. All interventions were home-based, being the principal one the rehabilitation, direct and indirect (training and advising the main caregiver. It was accompanied by psychosocial coping strategies aimed at controlling the patient's fear of the consequences and the risk of exhaustion of the caregiver. The main result is the improvement obtained by the interventions of the nurse and family caregiver, as evidenced by the change in the diagnoses and the positive development of NOC indicators at six months. Helping patients to regain their previous functional level as soon as possible and help them return to their social environment should be the essential objective in these processes. Moreover, their families should be included.
