Abstract

The need for replacement of the nasal dorsum is clearly demonstrated when used for open nasal roof, irregular surface, side collapse and valvular alteration after nasal hump resection. Anatomically requires a straight, symmetric and prominent osteocartilaginous structure achieved through the placement of a graft cartilaginous framework. It is simplistic thougth, that the hump resection alone is the universal procedure applyed to achieve a nasal dorsum straight, withouth considering height, width, length, surface regularity, valve function and optical perception of the nose. Cartilage grafts in gelatinbased haemostatic sponge Spongostan® (Johnson & Johnson), has been used experimentally for filling tridimensional defects in dynamic surfaces joints with positive results for chondrocyte differentiation and decrease of de-differentiation. We report our experience of almost 10 years with the use of this material as a mean of containment and tridimensional scaffold that improves the viability, stability and chondrocyte proliferation in replacement of the nasal dorsum in primary and multiple rhinoplasties. We also discuss their advantages, disadvantages and long term results.
