Background and Objective. Hand deffects represent a reconstructive challenge because the special caracteristics of this anatomical region, as well as the associated injuries that usually are present. The aim of this paper is to present our experience gained in recent years and to review the different reconstructive options. Methods. We present a serie of 17 cases with deffects affecting exclusively at the hand, both dorsal and palmar sides, that have required flaps for coverage. The choice of flap has been based primarily on the functional needs of the area to be covered, associated lessions and tmorbidity of the donor site. Results. From the total of our 17 cases, 11 were dorsal deffects and 6 were palmar deffects; 13 cases eventually required a free flap coverage. Conclusions. Free flaps represent the best option for reconstruction of deffects in hand for its variety of options and versatility to adapt to the specific characteristics of anatomical areas of the hand as well as the need of reconstruction for associated injuries.
