Life expectancy is increasing. In the Canary Islands, even with a population younger than the national average, the rate of aging is greater.
The Integral Geriatric Assessment (VGI) is considered the fundamental tool on which the geriatric clinic is based.
Since 2002, the Community Liaison Infirmary (ECE) in the Canary Islands has been carrying out a case management task with a VGI approach with the aim of improving the home care provided by the Primary Care Teams (EAP).
The ECE of Tenerife, have decided to agree on the performance criteria analyzing the work performed, to facilitate daily practice, and the development of tools to assess, assess and measure the results of case management, with the possibility of extending it to the rest of the ECE of the Canary Islands.
