Background and objectives:
Patient Care Report (PCR) collects nursing care, which is provided to the patient during the hospital stay. CONECTA 72 is a program to ensure the monitoring of these cares and, the PCR is the overarching element. Case Management Nurse focuses its activity on advanced practice of care. The objective is known the doctor's opinion about PCR, CONECTA 72 and Case Management Nurse in Health Area of Ferrol.
Methods:
Descriptive observational study with 172 specialists and residents. They participated voluntarily in the study through an ad hoc data collection notebook sent by email after the corresponding permits.
Results:
About 78.1% of them know the existence of PCR; 73.1% had read it occasionally; 88.6% believe it is convenient for patients to receive it when they left the hospital. About 61.6% of them know CONECTA72, although 23.2% have not participated in the program. 83.2% believed the figure of case management nurse is convenient.
Conclusion:
The PCR is an effective instrument to give continuity of care, but there is a significant lack of knowledge about when is performed and its content. It seems necessary to deepen the promotion of CONECTA72, since it is unknown by a third of the medical staff; although it does not seem that its existence can replace the figure of Nurse Case Manager.
