In Spain, since the demise of the classical teaching hospitals, the practical teaching of the degree of Medicine takes place, through formal agreements, usually in general hospitals of the National Health System. This procedure is not properly consolidated after 30 years. At the present we are witnessing a progressive disappearance of the personnel involved and difficulties in recruiting new clinical staff. The introduction of the Bologna model and the massive incorporation to clinical services of medical students from the high number of medical schools existing in Spain has created growing tensions to which responsible officials are not paying enough attention. Universities, schools, autonomic Departments of Health and the Health Ministry should analyze how clinical teaching is being done in teaching hospitals, evaluate results and establish measures, in a similar manner as it was carried with the postgraduate training, now considered the jewel of crown of Spanish medical training.
