Background and Objectives. We can't deny the excellent results obtained by the forehead flap in nasal reconstruction, becoming an important tool for reconstructive surgeon. Our aim is to present and discuss a modification performed to the paramedial forehead flap: a narrow pedicle, leaving the contralateral remaining forehead for another potential paramedial forehead flap if necessary. Methods. We analyze a group of patients who underwent skin nose basal cell carcinoma resection until clear margins were obtained by frozen biopsy, and afterwards, surgical reconstruction with a paramedian forehead flap with a narrow pedicle of 1.5 cm wide was made, based on the fact that supratroclear artery has a constant anatomic localization on the orbitary border. The donor site was closed primarily. Results. Seventy six patients underwent this technique: 30 female and 46 male, with an average age of 59 years old. All patients underwent revisions like transaction of the pedicle, flap defat, and cartilage graft. No flap necrosis was reported. Conclusions. The usefulness of the paramedian forehead narrow flap, and its advantages, were found. Among others, the possibility of maintaining the integrity of the opposite forehead side for another potential paramedian frontal flap if necessary.
