It is very common for the nursing staff to find out that the information flow regarding an elderly patient on admission is deficient or scarce. To resolve this problem and improve the attendance continuity of the elderly people when they are discharged or transfered from our Center Residence Clinic Geriatric "Los Robles", the nursing staff have developed an instrument for a better register that we have denominated "Nursing inform for the discharge or transfer to a resident for old people". In that inform we try to collect in a schematic, agile, and complete way the reality and the bio-psycho-social situation of the old people guaranteeing an adequate transmission of the information and the continuity of nursing cares. This instrument was created taking as a basis to a great extent Virginia Henderson's needs model (1). In it, a series of basic aspect of the elderly person state are collected, by means of a sequence of opened and closed items, including the basic state and the acute process, if they were any, and it is addressed to the health-care staff of the final center and to the care takers of the old person if they are translated to their own home.
