Currently, chronicity is a term that concerns society and specifically primary care nursing. It is true that the patient is no longer a passive actor, but rather that he has become the main protagonist of his care. The chronicity of our patients is rising to the creation of new strategies for their approach, and from this clinical case we want to present another vision of the assessment of people with chronic diseases.

Objective:

To carry out the nursing care plan we used a patient recruited from primary care, through the assessment using the middle-range theory of self-care of chronic illness.

Methodology:

The NANDA format was used in the making of the care plan, the indicators were established according to the NOC results criteria and the interventions were established using the NIC format.

Conclusion:

The patient with one or several chronic diseases requires the effort of primary care nursing to educate, guide and support. Trying to increase the empowerment of the patients and try to be part of the decisions in their health. All of this is going to reduce costs in the health system, reducing the care burden and improving care for complex disease processes.
