Patients admitted to Intensive Care Units (ICU) who require invasive mechanical ventilation increase the production of bronchial secretions, obstructing the airway. Therefore, endotracheal aspiration is necessary to eliminate them, an invasive procedure that improves permeability and oxygenation. There are two methods: Open Aspiration System (SAA) requires disconnecting from the fan and Closed System (SAC) without disconnection.

A bibliographic review is carried out, with the aim of finding scientific evidence that accredits the efficacy of the use of the SAC and the risks involved in adult patients with orotracheal intubation admitted to the ICU.

It is concluded that SAC can improve oxygenation (Recommendation IIB its use in adults with high FIO2 or PEEP, or risk of lung collapse), risk of infection and limits environmental contamination. The creation of a protocol is proposed to ensure compliance, indication and correct performance of the technique, patient safety.
