Osteoarthritis (OA) rank highly among global causes of disability and chronic pain. OA is also responsible for substantial health and societal costs, both directly and as a consequence of impaired work productivity and early retirement. Despite numerous efforts, treatments to paliate the impact of the disease have not reached a threshold of efficacy. We review the utility of opioids for the treatment of pain due to OA. Opioids provide no clinically relevant pain relief in the short- and intermediate-term for osteoarthritis pain. They provide a clinically relevant reduction in disability in the short but not in the intermediate term. The short- and intermediate-term tolerability of opioids is low. Therefore, opioids have a limited role in the management of OA pain in selected patients.
