Tunneled island flaps are a variant, whose aesthetic and functional results can be highly effective and efficient. They are effective because they achieve the correction of skin defects resulting from tumor lesions, traumas, etc., and they are efficient because they allow rationally use of the donor area, reducing iatrogenic effect and keeping the viability of reserving tissue for possible recurrence, as in cases of cancer, ulcers in extremities, among others. We present a series of cases describing our experience with tunneled island flap at the Hospital Universitario 'Hernando Moncaleano Perdomo ", a third level reference hospital y the south of Colombia, a geographic area with limited resources, serious conflicts and difficul postoperative follow-up of patients. We began our experience in 2009 with a case report published in this journal and until 2011 we have reviewed 35 cases of patients operated with defects in face, neck, armpit and legs. We emphasize on the possibility of using this type of flaps as a better alternative to face and neck defects reconstruction when health care resources are limited. Finally, a concise description of the techniques used and results obtained is conducted.
