Background and Objectives. Soft tissue reconstruction of complex defects of the thigh are rarely reported in literature. Different options to solve these complex defects have been described, including regional and free flaps. Patients and Methods. We present our clinical experience with 2 female patients, presenting un extensive postraumatic thigh defect, that was reconstructed using a free DIEP (Deep Inferior Epigastric Artery Perforator) flap. Results. After the procedure adequate correction was achieved and both patients were satisfied. We have only one thigh seroma in case number 2, that required surgical drainage. Conclusions. Thigh and lower leg reconstruction are common soft tissue defects, involving important contour deficit and unaesthetic appearance, and normally extensive and complex reconstructions are needed. We think that traidcional indications for the free DIEP flap, should to be extended, because in extensive soft tissue defects, this flap when is available, is in most cases the best choice, especially in women.
