Reconstruction of complex scalp lesions requires the use of surgical techniques that allow the closure of the lesion with tissues that have hair follicles to reduce the aesthetic impact in the final result. There are different surgical procedures, from primary defect closure, second-intention closure, skin grafts, local and regional flaps to microvascularized free tissue transfer. The use of tissue expansion and complex scalp flaps are useful tools when it comes to surgical planning and have considerably increased the chances of covering the lesion through local procedures. The primary objective of this type of techniques is to achieve coverage of the defect, obtaining acceptable functional and aesthetic results, with an individualized approach for each patient.

The objective of this paper is to present a case of complex scalp avulsion in a 15 years old patient, secondary to lift trauma and its reconstruction with cutaneous autograft of the avulsed segment, tissue expansion and locoregional flaps.
