Periodontitis is a multifactorial disease. Smoking is one of the environmental factors that most changes the susceptibility of the individual to the development of the periodontal disease. Analogous to what happens at the level of general health, tobacco causes serious adverse effects in the oral cavity, namely in periodontal tissues. The epidemiologic data taken from literature gives important information that is in agreement with the greater part of the criteria of Bradford Hill for the establishment of a causal relationship between tobacco and periodontitis. The pathogenesis of the periodontal destruction is extremely complex, since there are diverse critical pathways interconnected between them which participate in the outbreak and evolution of the disease. The mechanism of action of tobacco can be explained by its intervention in each of these pathways. In addition to being a risk factor for the disease, tobacco impairs healing and the response to basic and surgical treatment. Bibliographical revision of the published works related to the subject will be carried out.
