Nutritional optic neuropathies (NON) are usually chronic, progressive, painless and with bilateral involvement. Alcoholism and tobacco are the most common causes of toxic optic neuropathy (TON) which in often associated with a vitamin deficiency, constituting tobacco-alcohol amblyopia. This should be suspected in patients with malnutrition with heavy use of either or both of these substances and who have decreased visual acuity. Fundoscopy may reveal oedema or pallor of the optic nerve progressing to optic atrophy although it is usually preserved. Treatment is based on vitamin and nutrient supplementation, and the immediate cessation of smoking or alcohol consumption.
