Introduction:

In the Canary Islands, the case management nurse (CM) has been called the community liaison nurse (ECE).

Objective:

To evaluate the impact of the ECE model in terms of results for the patient and family, and efficiency outcomes for the health system. Identify evidence in QA.

Method:

The indicators are collected monthly, referring to home care and its impact on clinical results and use of resources. The comparison between trends in the indicators with and without the ECE model is carried out with Snedecor's F test. All these tests are bilateral at a level of significance p < 0.05. Scoping Review on CM.

Outcomes:

In the areas with ECE, improvements are obtained in the following indicators: 1) which describe the control of the patients' clinical situation; 2) the efficiency of the use of resources; and 3) the quality and compliance of the process, among which are also included those of home visits and the detection and approach to social risk. Twenty-one systematic reviews were selected. There is evidence in favour of CM in terms of effectiveness, quality of life, satisfaction and costs.

Discussion:

Where ECE work is carried out, improvements are obtained in the control of the clinical situation of the patients, quality and efficiency of care. The results are consistent with what has been published by other authors in different contexts and patient profiles.
