The increased life expectancy in patients with human immunodeficiency virus (HIV) leads to a higher prevalence of multi-pathology, polypharmacy and treatment burden at an earlier age than in general population. Potential problems associated with polypharmacy can affect adherence to the antiretroviral therapy regimen, a fundamental aspect in disease control, and increase the risk of drug interactions, adverse drug reactions, hospital admissions, falls and death. Therefore, the management of these patients is complex, requiring comprehensive care with a multidisciplinary approach that takes into account all the particularities of this population group, focused on improving their quality of life. We present the case of a 70-year-old man diagnosed in 2005 with HIV with multiple associated comorbidities, who was admitted to the hospital with suspected SARS-CoV-2 infection. During admission, the Hospital Pharmacy Service was consulted for a possible adjustment of pharmacological treatment, which proposed several recommendations to reduce the complexity of pharmacotherapy, as well as the profile of interactions, that contributed to positive resolution of patient's condition.
