Immediate breast reconstruction after modified radical mastectomy or subcutaneous mastectomy, returns to the patients their femininity, while creating a neobreast with adequate volume, symmetry, three-dimensional vision, anatomical shape and a tactile sensation in the reconstructed area. To this end, several surgical techniques have been described, among which we have used tissue expanders and anatomically shaped textured breast implants, with high profile and moderate height. We describe our surgical technique. When performing a mastectomy, we create a broad submuscular pocket, with no tension, conformed by the pectoral major muscle desinserted at its lower base, and a flap with medial pedicle of the external oblique muscle, which when sutured together, cover, protect and create a large pocket for the expander or breast prosthesis. Then, a skin-fat flap covers these flaps.
