So-called Saturday night palsy falls within neuropathies caused by the compression of a nerve when it passes through a canal or outlet and, in this specific case, the radial nerve is affected. The usual cause is having fallen asleep with an arm hanging down or compressed by the weight of the body. It is a clinical picture manifested by the inability to perform wrist dorsiflexion or to extend and separate the fingers. The usual evolution is towards recovery in 3 to 6 months. We report the case of a 40 year old male with this type of picture due to very severe compression of the posterior interosseous nerve.
