The pathophysiology of a bullet arterial embolization with the formation of an aortic ulcer can be a confusing and extraordinarily rare picture. It is evident to suppose severe vascular, cardiac or pulmonary damage as a consequence of the point of entry, which can lead to severe bleeding and thus represents a serious life risk, however it is surprising that this vascular trauma is associated with ischemia of a limb. In fact, these types of injuries have been rarely reported and their complications are often underestimated or are late diagnosed.

This rare event was described for the first time during the First World War, but now this phenomenon is usually associated with civil violence by firearm witch is gradually increasing.

In this document, we report a case that was treated in our center by thoracic endoprosthesis and surgical resection of the foreign body though arterial surgery.
