Anorexia nervosa is an eating disorder that often causes malnutrition and carries high mortality risk. A multidisciplinary and highly experienced team is needed to succeed in nutrition education and avoid the refeeding syndrome. We report the most severe case of malnutrition secondary to anorexia nervosa treated in our unit, a 33-year-old woman with a BMI of 8.8 kg/m² and high liver aminotranferases who did not experience any complication during the refeeding process despite the extreme gravity of her situation.
