The sural hemitriceps muscular flap is used in the National Institute of Orthopedics and Traumatology in Montevideo to reconstruct soft-tissue defects after severe lower extremity trauma on the middle third and the union of the middle and inferior third of the leg. The hemitriceps sural flap can successfully cover more extensive osseous exposure than the hemisoleus flap alone, with less surgical time and without affecting plantar flexion strength. A series of 13 cases is presented of leg open fractures with important cutaneous soft-tissue defect treated with hemitriceps flaps, operated on the period from march 1999 to july 2008. Average follow-up time was 15 months.
