We present a clinical case to be added to the apparently only 5 previously reported cases in similar clinical situations, identified after a systematic review of the available medical information network. This is a case of a massive size axillary cystic tumor, presented as a fluctuating mass in an adult patient, with an unknown original cause, that showed a continuous and asymptomatic growth. Final diagnostic was of massive axillary cystic lymphangioma. Although it's a rare occurrence in the adult patient population, a careful planning is a mandatory condition to obtain a satisfactory result. The most relevant aspect of our case was the need of a careful dissection to identify the afferent lymphatic vessels that should be individually ligated and transected to prevent relapse. All this information is valid too for congenital lymphatic masses, less frequent in this anatomical region compared with those appeared in adult patients.
