Current techniques for facial rejuvenation are the result of technical refinements after decades of good and bad results. Rejuvenation of the middle third of the face has recently regained interest due to the fact that it permits in-bloc repositioning of the ptotic malar tissues to its youthful state. An added advantage is nasogenian sulcus attenuation secondary to the vertical lift. To obtain this result there are multiple surgical techniques described. The latest surgical trends have shifted the main objective in classic rhytidoplasty from an ablative procedure to volumetric repositioning without tension. This paradigm shift renews the patient's own facial architecture giving them a more natural and pleasant result with decreased morbidity. We share a surgical option that offers a natural and harmonic facial rejuvenation by manipulating individual tissular vectors in order to reestablishing volumes threedimensional in the malar región and lateral face. This permits us to obtain a more youthful and long-lasting result without the undesired face against the wind effect that produce other rhytidoplasty techniques. Additionally it treats the frontal and cervical regions in the same procedure contributing to a complete facial rejuvenation.
