Secondary cleft lip reconstruction is a very demanding reconstructive procedure, and our goal must be to achieve a complete restoration of lip function and appearance. The workhorse for secondary cleft lip repair is the Sbattini-Abbe flap. Several modifications to the original technique have been proposed to adress lip deformities found among these patients, which are not only found in the central segment of the upper lip, but in the lateral portions of the lip, and both in the vermillion and the cutaneous lip. We present a modification to the original Sabattini-Abbe flap, with the aim of reconstruction of all defects with a single flap and adhering to Plastic Surgery principle of recontruction of like with like.
