Necrosis due to vasoconstrictors is a rare but very serious complication that can seriously affect the functionality and quality of life of patients. Plastic Surgery plays a very important role in the treatment and reconstruction of affected extremities. The individualization of each case is essential to make the best surgical decisions.
We present the case of a patient who, after septic shock of urological origin, developed acral necrosis of all 4 limbs and was reconstructed in our center with free flaps, introducing the concept of their reuse to reduce associated morbidities. Two superficial circumflex iliac artery perforator (SCIP)-based flaps were used to reconstruct the fingers of both hands and the soles of both feet.
We explain the key steps of each surgery and the patient's final result after a year and a half of follow-up.
