Reconstruction of defects in the groin-scrotum and pubic region is complex due to their anatomical and functional characteristics. There is no consensus of the reconstructive route and several strategies and algorithms have been proposed which are based on the size of the defect, the surgical experience, and the available resources.
The aim of this case report is to include the keystone flap in the reconstructive route of defects in the pubic and groin-scrotum region, highlighting its advantages, the versatility in the reconstruction like-to-like, the surgical time, its vascular reliability, the appropriate closure of the donor area and the recovery time.
We present the case of a young-male patient with a history of an arteriovenous malformation in the pubic-scrotal region since birth with indication for surgical resection. A coverage defect is obtained with involvement of the 90% of the pubic region, the skin at the base of the penis and the right scrotal region. Reconstruction was carried out independently by subunits using local flaps for the scrotal sac and the base of the penis, the pubic defect was reconstructed with a keystone type IIA flap. We obtained 100% coverage of the primary defect and the secondary defect of the donor area. The case is analyzed considering the available scientific literature.
