Open fractures of the calcaneus represent high risk of infection, sometimes leading to complete removal of the bone. The scenario of a total calcanectomy becomes a great surgical challenge, since it is not only necessary to solve the residual covering defect, but also to recover the gait function, because this bone has a fundamental role in human walking due to its capacity to support weight and compression.
We present the case of a patient who, after an open calcaneal fracture, develops osteomyelitis that persists despite treatment, so a total calcanectomy is performed. For reconstruction, a fibula osteocutaneous flap is performed, introducing a novel form for placement in place of the calcaneus, which allows the limb to be saved. After 6 years of follow-up, patient achieved total restoration of walking.
