The outcome of children with brain tumors has improved in last years, especially for some histological types, early stages or after performance of radical surgery. However, patients with high risk tumors continue to experience a dismal prognosis with conventional treatment. In an attempt to improve our outlook on these patients, new strategies utilizing high-dose chemotherapy (HDCT) with stem cells rescue have been developed. After the first studies conducted in patients with recurrent tumors, the procedure was extended to newly diagnosed primary tumors, and then presently to young children in an attempt to avoid brain irradiation. The analysis of the results obtained by different groups in recurrent tumors, the selection of patients, and the tumors benefiting from the treatment, complete this paper about the role of high-dose chemotherapy in children with malignant brain tumors.
